Background Check Acknowledgement

As part of the application and onboarding process for becoming an Elevator (mentor) with Village Elevate, I acknowledge and understand the following: I will be required to successfully complete a background check before being granted access to work with families and children on the Village Elevate platform. This background check will be used to verify my identity and screen for any criminal history or other factors that may impact my eligibility to serve in a mentorship capacity. I consent to the collection and use of my personal information for the purpose of conducting the background check in compliance with applicable federal, state, and local laws. I understand that failing to complete or pass the background check may disqualify me from participating as an Elevator on the Village Elevate platform. By submitting this acknowledgment, I certify that the information I provide is true and accurate to the best of my knowledge, and I voluntarily agree to participate in the background check process.
